FIRST-YEAR/TRANSFER/FIRST-TIME STUDENT-ATHLETES


Medical Packets: Prior to participating on a Sarah Lawrence College team, student-athletes must submit a completed medical packet which includes a current physical*, assumption of risk, emergency contacts, primary insurance information, and health history information. To expedite this process, SLC uses an online data entry system called Athletic Trainer System (ATS). Instructions to create an account are below. Once you have created your account, log in and complete all required information. *Current physical must be dated within 6 months of your team's first practice. If you are a first-year or new transfer student, we can accept the same physical you submitted to Health & Wellness provided it is dated within 6 months of your first practice and indicates that you may participate in sports with no restrictions. 

Proof of Sickle Cell Status: All First-Year/Transfer/First-Time student athletes must submit proof of their sickle cell trait status.
Concussion Management (ImPACT Testing): All NCAA members are required to have a concussion management plan in place. To that end, SLC Sports Medicine utilizes the ImPACT system for baseline and post-injury assessments. The baseline test is administered on campus prior to a student-athlete's first and third season of participation.


RETURNING STUDENT-ATHLETES

Medical Packets: Medical information must be updated annually on ATS for student-athletes who previously participated in a varsity sport at SLC. Returning student-athletes must also submit a current physical* before beginning practice. The physical form can be found below and should be uploaded into ATS. We can also accept your doctor's form provided it indicates you may participate in sports without restriction. *Current physical must be dated within 6 months of your team's first practice.

Concussion Management (ImPACT Testing): All NCAA members are required to have a concussion management plan in place. To that end, SLC Sports Medicine utilizes the ImPACT system for baseline and post-injury assessments. The baseline test is administered on campus prior to a student-athlete's first and third season of participation.

*UPDATED May 2024* Participation in Athletics for Transgender Student-Athletes: At its January 2022 meeting, the NCAA Board of Governors updated the transgender student-athlete participation policy governing college sports. NCAA Medical Exception Procedures require that the use of anabolic agents, hormone and metabolic modulators, or peptide hormones, growth factors, related substances and mimetics must be approved by the NCAA before the student-athlete is allowed to participate in competition while taking these medications. Transgender student-athletes must contact the Director of Athletics or Head Athletic Trainer to review documentation requirements.  
Medical Exception for ADHD: The NCAA requires that all athletes on stimulant medication for the treatment of ADD/ADHD provide adequate documentation of diagnosis and treatment to allow for a medical exemption. Student-athletes currently prescribed stimulant medications should upload the completed form for ADD/ADHD medical exception to their ATS account.
