As a Sarah Lawrence College student-athlete, it is your obligation to report ALL illnesses, medical conditions and injuries to the athletic training office immediately. Failure to do so may result in loss of insurance coverage.All student-athletes are required to sign an acknowledgement of these rules prior to participation. SLC may be unable to provide treatment or insurance documentation if these procedures are not followed.
INSURANCE CHANGESStudent-athletes are required to provide a copy of their current medical insurance card(s) prior to participation. If your insurance situation changes at any time, it is your responsibility to submit an updated copy to the Athletic Training staff.
IF YOU ARE TREATED OFF-CAMPUSAny time you seek an outside provider for medical services, you must provide medical documentation clearing you for athletic participation.
In other words, if you see your personal physician for illness or an injury, it is the obligation of our Athletic Trainer to withhold you from participation until that physician provides documentation permitting you to return to play.
SEEKING OUTSIDE TREATMENTIn the event that an injury incurred in conjuction with SLC athletics results in the need to seek outside consultation, you must
advise the athletic training office BEFORE making an appointment. This is the only way to ensure that proper insurance procedures are followed.
For most student-athletes, the department's insurance functions as a secondary carrier. You are required to remit any outstanding medical bills and Estimations of Benefits from your primary insurance carrier.
ISSUED EQUIPMENTAny equipment issued to you by the department of athletics (crutches, ankle braces, etc.) must be returned when the equipment is no longer necessary, your playing season has ended, or you leave the College. You will be financially responsible for any lost, stolen or damaged equipment.
